the short version
  • If I lost access tomorrow, my weight would come back. The research says that's normal.
  • No one on a GLP-1 has a forever guarantee, compounded, covered, or cash pay.
  • You can be scared and still be okay today.

If I lost access to my GLP-1 tomorrow, I would gain the weight back.

Not some of it. Not "maybe if I'm not careful." I know exactly what happens, because I've felt it every time I've gone without. The cravings come back. The food noise comes back, all of it. I have way less control over what and how much I eat, and it isn't subtle.

I don't think I'm at a place where I could keep the weight off without it. I'm saying that out loud on purpose, because I think a lot of you feel the same way and have never told anyone.

The first headline scared me. The fifth one didn't.

My medication is compounded. When the first wave of "compounded GLP-1s are going away" news hit, I panicked. I remember thinking, this is it. This is where it ends.

And then it didn't end. And the same headline came around again. And again. By about the fifth time, the fear got quieter.

the fear didn't leave. it just stopped running the show.

It helps that I'm close with the people at the company I use, and I trust I'd hear early if something were about to change. My logic is simple: if they're not worried, I'm not worried. But I know not everyone has that, so here's where things stand in plain English. (This is not legal or medical advice, just my best read of the news.)

  • The big FDA change is still a proposal. In April 2026 the FDA proposed keeping semaglutide, tirzepatide, and liraglutide off the list that lets large outsourcing facilities compound them from bulk ingredients. The comment period closed in July, and as of late October there was still no final decision.
  • It mostly targets the big facilities. Pharmacies that compound for one patient's prescription at a time are already tightly limited and generally need a documented medical reason for a version that's different from the brand name.
  • Insurance isn't a guarantee either. Fewer employers are covering GLP-1s for weight loss this year than last, and some have already dropped it.

Whether yours is compounded, covered, or cash pay, nobody on a GLP-1 has a forever guarantee. I'm not saying that to scare you. I'm saying it so you know that if you feel this, you're not being dramatic.

Why I'm so sure it would come back

I don't need a study to tell me. I've lived it. But if anyone has ever told you you'd "just need to keep up your habits," here's what the research actually shows:

  • Most of it came back within a year. In a big semaglutide trial, people regained about two-thirds of the weight they'd lost in the year after stopping.
  • It comes back faster than after a diet. A 2026 Oxford review of 37 studies found weight came back about four times faster after stopping medication than after diet and exercise programs.
  • The food noise comes back too. Doctors who treat obesity say the constant food thoughts usually return after stopping, and that hunger is closely tied to the regain.

That isn't a willpower problem. It's what happens when the medication isn't in your body anymore. If you've been quietly afraid you'd "fail" without it, you're not imagining things. You're describing what happens to most people.

Needing it isn't failing

Nobody stops their blood pressure medication, watches their blood pressure go back up, and calls themselves weak. Nobody calls that a lack of discipline.

But with weight, we've been trained to think the medication is training wheels. Like the real win is getting off of it and white-knuckling the rest.

I don't buy that anymore. Maybe one day I'll be able to come off of it and keep the weight off. Maybe I won't. Right now, I need it. And that doesn't mean I did anything wrong.

What I actually do with the fear

I'm not going to hand you a 10-step plan, because I don't have one and fear doesn't work like that. Here's what's true for me:

  • I worry when there's something real to worry about. A scary headline isn't the same as my medication disappearing tomorrow. I keep an eye on the news, trust my provider, and go live my life.
  • I keep my systems going. Walking, protein, and eating from a plan built around food I actually like. Not because it would replace the medication. It wouldn't. But I'd rather have systems than nothing.
  • I don't let fear push me into something sketchy. If you ever lose access, talk to your prescriber before you buy anything online that claims to be the same thing for cheap. "Research grade" is not something you want to inject.

If you're curious where I get mine, here's more about Elara. Prescribing is always up to their providers. (Affiliate link.)

You're allowed to be scared and still be okay today. Both can be true at the same time.

Quick note: I'm not a doctor. This is my personal experience, not medical or legal advice. Please talk to your prescriber about anything related to your medication.
